The Occidental College Sports Medicine department uses a web-based software program called The Athletic Trainer System (ATS) for the maintenance of our medical records. All of your medical clearance forms are available to you through ATS. Below is a link to an instruction document and checklist that will walk you through the steps of setting up/updating your ATS account, accessing your medical clearance forms, and completing the medical clearance process.
All Oxy athletes are required to obtain an annual physical exam with their primary care physician during the summer break. Be sure to print the Physical Exam Form (accessible in ATS) and take it with you to your physical exam appointment. Your physician will need to SIGN and STAMP at the bottom of the Physical Exam Form. You will then need to upload the completed form back into ATS. Step-by-step instructions on how to do this correctly are explained in the instructions document below, so please be sure to read through it.
We cannot accept Physical Exam Forms that do not include the physician’s SIGNATURE and STAMP. It is your responsibility to make sure the office stamps the form.
The NCAA requires that the Physical Exam Form must be signed by a MD or DO. Forms signed by nurses, chiropractors, or other providers will not be accepted.
HEALTH HISTORY FORM:
One of the forms you will be required to complete in ATS is the Health History Form. It is important that you provide detailed explanations (including dates whenever possible) for any questions that you answer “yes.” Lack of explanations or insufficient explanations may delay your clearance process.
If you use an asthma inhaler, be sure to obtain an extra inhaler from your doctor to give to the Oxy sports medicine staff. You will also need to obtain specific documentation from your physician explaining the necessity of your inhaler. Please see the instructions document for more information.
